Case Management
New Leaf on Life provides case management and peer-to-peer supportive services to Transitional Aged Youth, adults, and vulnerable communities. We offer a range of direct services to individuals and families in the community of Los Angeles County where there are over 75,000 unhoused individuals. Some of the main services we provide include mentorship, life skills training, substance abuse support groups, wellness support, and access to education and career development.
